Read the following release, fill in the necessary information, and sign by typing parent/guardian initials in the box. Chaperones that are staying in the dorm and/or eating in the lodge must register separately. Minor students WILL NOT be accepted for registration without a medical release!
I understand that there are risks of physical injury while participating in camp activities. Montana Fiddle Camp will do its best to minimize these risks. I understand that Montana Fiddle Camp does not carry health insurance covering illness or injury for the participants in the program and I have included the name of my insurance company on this application. I hereby authorize the staff or volunteers of Montana Fiddle Camp to use their best judgement to obtain medical treatment for my child in any emergency. I hereby authorize any required medical services for my child, and agree to be responsible for the payment of those services.

Registration & Refund Policy:
When we receive your registration, you will receive a confirmation letter either by email or mail. If you do not receive the confirmation letter within two weeks, contact the camp secretary to confirm that she has received your registration and deposit. Deposits are non-refundable after March 15, 2026.
